Immunization Information

This vaccine requirement can be met through one of the following:

• Two doses of MMR vaccine (given after your first birthday)
• Two doses of Measles and one dose of Mumps and one dose of Rubella vaccine (given after your first birthday)
• Medical documentation of history of disease(s)
• Blood titers showing immunity (lab report required)
• Born before 1/1/1957

NYS Public Health Law 2167 became effective January 1, 2017. This law requires all colleges/universities to distribute information about meningococcal disease and vaccination. For more information regarding New York State PHL Section 2167 meningococcal disease and vaccinations please see meningococcal disease fact sheet. According to the NYS Public Health Law 2167 every student is required to address the meningitis requirement. To meet this requirement provide proof of vaccine given in last five years or decline the vaccine through My Hazen the patient portal

• Proof of at least 1 dose of meningococcal ACWY vaccine (Menveo, Menactra or MenQuadfi)
• OR a complete 2 or 3 - dose series of Meningitis B (Trumenba or Bexsero) vaccine
• OR complete the waiver form refusing meningococcal vaccine which can be found on My Hazen under the “Forms” tab in the box labeled “Immunization Communication”. **

**If you are under the age of 26 and living in the residence halls, it is strongly recommended that you be vaccinated against meningitis and do not waive this requirement.

Meningococcal ACWY Vaccine: What You Need to Know

Meningococcal B Vaccine: What You Need to Know

All new (including transfer) students at SUNY Brockport must complete a screening questionnaire for tuberculosis risk. The Tuberculosis Screening Form needs to be completed and submitted online through the MyHazen patient portal.

• If you have previously tested positive for tuberculosis, you DO NOT need to have another tuberculosis test. You MUST upload the records for the treatment of your tuberculosis through MyHazen.
  • Go to the UPLOADS tab and select Immunization/TB Records.
• If you received a secure message through MyHazen indicating that you need a tuberculosis test, please see your medical provider to have a tuberculosis test completed.
  • The Health Center Staff will provide you further information regarding testing in a secure message through MyHazen.
  • Once completed, access MyHazen to UPLOAD the laboratory report to Immunization/TB Records.

Tuberculosis Testing

If you require further testing, it should be completed prior to your arrival at SUNY Brockport.

Domestic Students

SUNY Brockport Health Center Staff will review the Tuberculosis Screening Forms. If further testing is required, we will reach out to the student through the MyHazen patient portal (secure messages).

Positive Tuberculosis Test Results

If you have previously tested positive for tuberculosis, you DO NOT need to have another tuberculosis test. You MUST upload the records for the treatment of your tuberculosis to MyHazen. Go to the UPLOADS tab and select Immunization/TB Records.

If you have never tested positive for tuberculosis previously, you must have a chest x-ray performed within the 12 months prior to enrollment. This is done to ensure that you are not able to spread active tuberculosis to others. This MUST be completed PRIOR to arrival at SUNY Brockport.

You are required to comply with NYS Immunization requirements for secondary education.

New York State requires students to provide immunization records by the first day of classes.

Measles, Mumps and Rubella (MMR)

This vaccine requirement can be met through one of the following:

• Two doses of MMR vaccine:
  • First dose given no more than 4 days prior to the student’s first birthday.
  • Second dose at least 28 days after the first dose.
• Two doses of Measles and one dose of Mumps and one dose of Rubella vaccine (given after your 1st birthday).
• Medical documentation of history of disease(s).
• Blood titers showing immunity (lab report required).

Meningitis

According to the NYS Public Health Law 2167 every student is required to address the meningitis requirement.

To meet this requirement, provide proof of vaccine given in last five years or decline the vaccine through My Hazen the patient portal.

• Proof of at least 1 dose of meningococcal ACWY vaccine (Menveo, Menactra or MenQuadfi) in the last 5 years.
• OR a complete 2 or 3 - dose series of Meningitis B (Trumenba or Bexsero) vaccine.
• OR complete the waiver form refusing meningococcal vaccine which can be found on your MyHazen patient portal under the “Forms” tab in the box labeled “Immunization Communication”. **

Tuberculosis (TB) Screening Process

All students must complete the “Tuberculosis Screening for International Students” Form on your MyHazen patient portal under Forms.

Students Arriving in the U.S Summer of 2026

Students from high-risk countries will be screened for Tuberculosis upon arrival to SUNY Brockport. This will be completed at the Student Health Center by obtaining a Tuberculosis Blood Test (also called interferon-gamma release assays or IGRAs) such as:

• QuantiFERON-TB Gold Plus OR
• T-SPOT TB Test (T-Spot)

IMPORTANT: Vaccination with live viruses, including measles, mumps, rubella (MMR), oral polio, varicella, and yellow fever may interfere with the TB blood test (QuantiFERON/IGRA) reactions. When scheduling your immunizations, please be advised it may impact your TB blood test once you arrive to SUNY Brockport

• Live-virus vaccines such as MMR should be administered 4-6 weeks prior to arrival at SUNY Brockport.

If you have previously tested POSITIVE for Tuberculosis, you MUST upload the records of your positive IGRA/QuantiFERON to your MyHazen patient portal under “Immunization/TB Records.”

POSITIVE TB Blood Test RESULT:

• Must be an official lab test report.
• Must be in English and include your name and date of birth (English format).
• Must include the date of testing.
• Provide medical documentation along with a NEGATIVE chest x-ray. Upload your results to your MyHazen patient portal.
• If you have already received a full course of treatment for TB, you must provide medical documentation confirming the start and end dates of treatment and the medications prescribed. Upload your documentation to your MyHazen patient portal under selection “Immunization/TB Records.”

Students Currently in the U.S (Born in High-Risk Countries)

Students from high-risk countries need to be assessed for Tuberculosis.

• PPD tests/Tuberculin Skin Test are not accepted.
• Submit Tuberculosis Blood Test (also called interferon-gamma release assays or IGRAs) such as:
  • QuantiFERON-TB Gold Plus OR
  • T-SPOT TB Test (T-Spot)

TB Blood Test RESULT:

• Must be completed in a U.S. lab.
• Must be an official lab test report.
• Must include your name and date of birth.
• Must include the date of testing.
• If your test is POSITIVE, you must get a chest x-ray as soon as possible and upload your results to your MyHazen patient portal under selection “Immunization/TB Records.”
• If you have previously tested positive for TB, you must provide medical documentation along with a NEGATIVE chest x-ray. Upload your results to your MyHazen patient portal.
• If you have already received a full course of treatment for TB, you must provide medical documentation confirming the start and end dates of treatment and the medications prescribed. Upload your documentation to your MyHazen patient portal under selection “Immunization/TB Records”.

Current International Students from High-Risk Countries

The Monroe County Health Department (MCHD) has provided updated Tuberculosis guidance for students traveling outside of the U.S. to high-risk countries.

• Re-screening is recommended for those traveling to high-risk TB countries upon return to the U.S.
